Health Empowerment Zone
Emergency Patient Care (EPC) is an emergency contact program to make access to health information more equal for diverse and disadvantaged populations. EPC helps solve the problem of people not having access to their personal health care information in the event of a medical emergency or major disaster.
By registering in the EPC program, necessary emergency personnel, and/or those emergency contacts you select, will have access to your personal health information 24/7. Simply the toll free telephone number listed on your EPC card. By reaching our 24 hour call center, they will speak to friendly agents ready to forward your personal health information, enabling the emergency personnel to better treat your needs or to inform them of whom you have listed as an emergency contact.
- Click on the EPC link to begin registration of your secure Electronic Health Record. You will be redirected outside the National Coalition website.
- Click on the EPC Registration link next to the log in box. A new screen will appear.
- Fill out all of your information. List the National Coalition as your Institution. And click Register.
After registering, you will receive an email with your EPC user ID and temporary password.
Return to the National Coaliton Health Empowerment Zone and click on the EPC symbol. - Enter your user ID and temporary password, for security of your medical data. And click Log In. *Note: your email address is your user ID.
- Change your password to one of your choice. Passwords must be at least 7 characters long with at least one letter and one number (example:
“student1”).
This will complete your activation. You will enter into your Electronic Health Record. - Begin entering basic health information by clicking on XLMD Health.
In your file will be a tutorial on how to navigate your medical information.
You have health education tutorials in your file. Open the link for EPC Health
Education and begin learning over 3000 health topics.
If you forget your password, go to the log in page and click Forgot your password? Your password will be emailed to you.
